of the COC and endorsement
to it;
- Photocopy
of the GMDSS certificate
and endorsement to it (if
applicable);
- Photocopies
of all certificates required for
the current rank;
- Photocopy
of the seaman's book;
- Photocopy
of the international passport;
- One
photo for the application
form;
- Sea
service reports (if available).
